Canadian investigators went beyond the EPCAT II trial (1) and presented the EPCAT III study at the Congress of the International Society on Thrombosis and Haemostasis (ISTH) 2026 in Paris. The double-blind randomized trial, simultaneously published in the New England Journal of Medicine, compared antithrombotic prophylaxis with aspirin (ASA) alone versus rivaroxaban followed by ASA after hip or knee arthroplasty (2).

The METRO study (MEsoglycan for the secondary prevention of superficial vein ThROmbosis) was presented at the ISTH 2026 Congress by Prof. Giuseppe Camporese. The study, simultaneously published in eClinicalMedicine (Lancet), evaluated one year of treatment with mesoglycan versus placebo after resolution of an acute episode of superficial venous thrombosis (SVT). The research documented an approximately 30% reduction in recurrent SVT compared with placebo during the year following treatment discontinuation. The Italian study also provided the first prospective data on the natural history of SVT, reporting a 39% recurrence rate at two years in the group that received no preventive therapy after resolution of the acute thrombotic episode (1). We discussed these findings with Prof. Camporese (video).
